What to Expect at Your First Visit
You will be evaluated by a Board Certified Practitioner who will review your functional goals and plan in detail. This conversation and assessment will help us help you. If you have uncertainty or questions about the process, our experienced staff is here to provide answers and direction.
Please bring a valid prescription, your insurance card(s), and a photo ID with you to your appointment. During your visit, we will gather information from and about you. This includes details, such as your height, weight, age, prescribing physician, and medical history. If you would like to complete our registration process ahead of time you may utilize the below links. Step 1 can be completed online through our interactive online form and Step 2 may be printed, filled out and brought to our office at your scheduled appointment.
Some insurance companies require that we obtain an authorization before delivering your orthosis or prosthesis. Our administrative staff will start the process and submit the authorization request to your insurance on your behalf. Once the approval is received, you will be contacted to make an appointment to return for the delivery of your device.
