Please bring a valid prescription, your insurance card(s), and a photo ID with you to your appointment. During your visit, we will gather information from and about you. This includes details such as your height, weight, age, prescribing physician, and medical history.

Some insurance companies require that we obtain an authorization before delivering your orthosis or prosthesis. Our administrative staff will start the process and submit the authorization request to your insurance on your behalf. Once the approval is received, you will be contacted to make an appointment to return for the delivery of your device.
